what link do I have to refer the payment icon so it will refer to paypal for example 31 days for $5

I know that, sorry I wasn't very clear.
I mean what code do I need to refer my own made icon to ? I don't want to use the original table because I have made a full premium template with own made payment icons but I don't know which code to use so it will work with the script.

please could I get an example with for example an image linkin to 3 month premium for 15 dollar.
